Looking back on it, the clues had been obvious. The tilt of the 'h' in the note, the specific wording used, the method of requested payment. But Sherlock hadn't seen it. Hadn't deduced it properly. Hadn't solved it in time.
Lestrade put a hand on his shoulder, holding him back. "Don't in there, Sherlock. You shouldn't see him, not like that. He did best, even created a shelter out of cardboard boxes but…."
But the insulation they would have provided would have been insufficient for three days spent in an industrial freezer.
John had died.
And it was Sherlock's fault.

Oh my God, you killed John! You -- wait. Industrial freezer? You know, there's a greater chance of resuscitation if the body is found in a very cold environment, like a frozen river.
