Of course, the Doctor knew immediately that something was hiding behind Turlough’s innocent exterior. It had taken less than a second’s glance to see this wasn’t just some lost school boy who’d wandered into a transmat beam by accident.
No, there had been nothing accidental there. Nothing innocent either. Perhaps not an outright miscreant, but close. There was something, something hidden about him that the Doctor needed to discover. And what better way to find out what it was than invite the boy along when they left Earth? There was a mystery there, one the Doctor couldn’t wait to unravel.

I wonder how much had been preplanned by the writers. He talks about being stuck on Earth in Mawdryn Undead, but it's not till 9 adventures later that it's revealed he's not human.

Of course the Doctor knew Turlough was hiding something when he met him, he just didn't let on that he knew. Telepathic ship, remember - she'd know straight away.
